CPLR 8019 (C) Notification to Amend Docket
If a previously entered Judgment is affected by a subsequent order,
this form must be submitted by counsel to provide notice to the County Clerk pursuant to CPLR 8019 (c).
Be sure to include a copy of the order with this form.
